Athletic Physicals

Attention Athletic Parents!

Students MUST have a current physical to participate in athletics. There is an opportunity to receive a free physical at Christus Trinity Mother Francis on April 22nd at 8:00am sharp.

Students entering the 7th, 9th and 11th grade will need to get a new physical done. All other students may keep their current physical as long as it is no more than 2 years old.

When you bring the athletes to the mass physical event at CHRISTUS-Health Park Plaza on April 22 please make sure each student has the following:
UIL Physical Form with Health History filled out and signed by a parent/guardian. (attached)
2. Shorts, T-shirt, tennis shoes.
